SERVICES
Community Health Access hosts Educational Seminars to help individuals within the community make informed decisions on healthcare. Our team of knowledge educators especially enjoys helping seniors and values making a difference in their lives.
ACCESS
Through our partnerships with Access Centers we assist individuals with applying for Medicaid, Low Income Subsidy (LIS), Food Stamps, Free Phones, and Free E-Passes. We offer a multitude of services and resources.
PARTNERS
Community Health Access enjoys partnerships with other community and governmental organizations that share similar values and goals in promoting increased health and quality of life. We strive to increase resources for seniors through our engaged program partners.
ABOUT US
Community Health Access's purpose is to work with public and private healthcare stakeholders to develop, promote, and implement strategies to improve community health access through leadership development, education, and management services.
CHA helps form potential partnerships between for-profit and not-for-profit organizations, including government agencies, community organizations, and local businesses. These partnerships work to identify resources available from provider organizations, health plans, and community organizations to finance and promote programs that help individuals in the community.
CHA and its volunteers participate in the Stamp Out Hunger program in conjunction with the 501c3 organization, the Daily Bread Distribution Center. The Stamp Out Hunger program is sponsored by the US Postal Office Union and is the largest distribution program in the country. www.dailybreaddc.org
CHA volunteers distribute 1,000 bags of non-perishable food items each month. Since 2018, the food items have been purchased from and provided by our 501c3 partner, Advanced Community Outreach Center. www.advanceseniorcenter.com
CHA partners with the AARP Foundation program to have AARP participants help individual community members inside the Access centers. These AARP participants assist the Access centers in helping individuals apply for Medicaid, LIS, food stamps, and free phones along with providing access to other local, state and federal programs. www.youraccesscenter.com
